Establishing Medical Foundation for the Life Care Plan
The presentation will review published life care planning standards and consensus statements relative to seeking, gathering and establishing medical foundation for purposes of developing an evidence-based life care plan. Case scenarios will be utilized to illustrate appropriate and inappropriate methods for defining necessary life care planning medical foundation. Finally, the implications and risks inherent to breaching one’s professional scope of practice during the development of a life care plan will be addressed.
